You can obtain karamja rum from the quest "Rum Deal". You will need to talk to the Bartender in the Port Sarim Inn. When speaking with him, select the "Fancy a drink?" option and give him 20 gold pieces. He will then brew you a cocktail, and you will receive a Karamja Rum.
