How do you get a copy of your food service sanitation Illinois?

Option 1: Online

1. Go to the Illinois Department of Public Health (IDPH) website food safety website.

2. Click "Food Service Establishments" under "Program Areas" in the left navigation menu.

3. Click "Establishments" under the "Links to Commonly Accessed Topics" section.

4. Click on the county where the food service establishment is located.

5. Click on the name of the food service establishment.

6. Click on the "Inspection Reports" tab.

7. Click on the "Food Sanitation Inspection Report" link for the inspection date you want to view.

Option 2: By Mail

1. Send a written request to the IDPH. The request must include:

* The name of the food service establishment

* The address of the food service establishment

* The date of the inspection

* A $10 fee

2. Mail the request to:

3. The IDPH will send you a copy of the food sanitation inspection report within 10 business days.

Option 3: In Person

You can also pick up a copy of your food sanitation inspection report in person at the IDPH office in Springfield, Illinois. The office is open from 8:30 a.m. to 4:30 p.m., Monday through Friday.
